Your Privacy Choices

This page lets you exercise the privacy choices available to you under California’s CCPA/CPRA, Washington’s My Health My Data Act, Nevada’s consumer health data law, Connecticut’s Data Privacy Act and similar laws in other states. It works alongside our Privacy Policy, which describes in full how we collect, use, and protect your information, and our Consumer Health Data Privacy Policy.
ForbiddenRx does not sell your personal information or consumer health data, and we do not use your protected health information (PHI) for advertising. Even so, you may exercise the choices below at any time, and we honor them regardless of whether a “sale” occurs.
Do Not Sell or Share My Personal Information
Some website analytics and advertising technologies (such as cookies or pixels) may be considered a “sale” or “sharing” of personal information, or use for “targeted advertising,” under certain state laws — even when no money changes hands. You can opt out of this activity. We do not knowingly sell or share the personal information of anyone we know to be under 16.
Your Other Rights
Depending on your state of residence, you may also have the right to:
- Access or receive a copy of the personal information we hold about you
- Delete your personal information
- Correct inaccurate personal information
- Get a list of every third party and affiliate we have shared your consumer health data with, and how to contact each one
- Withdraw your consent to our collection and sharing of your consumer health data
- Limit the use and disclosure of sensitive personal information
- Be free from discrimination for exercising these rights
Health information you share with a provider through the Service is protected health information (PHI) governed by HIPAA and your provider’s Notice of Privacy Practices — see our HIPAA Notice. Requests involving medical records may be routed to the treating provider.
Make a Request
Use the form below to submit your choices. You do not need an account. We will verify your identity using the email associated with your request before acting on it, and an authorized agent may submit a request on your behalf with proof of authorization. We respond within 45 days (extendable once by 45 days, with notice), free of charge up to twice a year. If we decline a request you can appeal it, as described in our Consumer Health Data Privacy Policy.
